ANSWER

Transitioning patients from one area of care to another is a collaborative effort. Interprofessional collaboration is promoted within my facility by discharge planning from admission. SBAR is a tool that is enforced to create clear and concise communication to aid in transitions of care. In the intensive care units, multidisciplinary rounding is done at least once a shift.
