Collaboration Café: Reflect on Knowledge

Step 1: Reflect on your current health assessment skills and understanding of the importance of health assessment in client care.

Step 2: Answer all questions below with explanations and detail. No scholarly sources are required.

Paragraph one: Briefly describe the type(s) of health assessments you typically perform now or have performed in the past. Explain the setting and why the type of assessment was appropriate.

Paragraph two: Describe a personal experience where effective health assessment skills significantly impacted your client’s care. Discuss what you learned about the importance of health assessment from this experience.

Paragraph three: Share your thoughts on the importance of accurate and thorough documentation of health assessments. Describe the situation where documentation was crucial in client care or communication among healthcare providers.

SOLUTION

I currently work as a registered nurse on a medical-surgical floor, where performing thorough health assessments on assigned patients is imperative. I conduct ongoing or partial assessments with assigned patients, as their immediate problems have been assessed during admissions. I reassess these issues to ensure they are resolving and not deteriorating.
